Initial Consultation - Existing Patient previously seen at alternate facility: $159.00

• Establishing care to include one hour initial consultation catered around health history, health concerns, symptoms, aches, pains or health frustrations.  Also includes blood draw and patient history transfer.  

Annual Physical: $199.00

Acute Illness Visit: $199.00

Acute visit 15 Minute Consult in Office: $50.00

• Acute concern or health issue that you would like a 15 minute consult on.

Acute visit 30 Minute Consult in Office: $100.00

• Acute concern or health issue that you would like a 30 minute consult on.

Follow up Visit: $159.00

• This is a visit with a provider for follow up on lab work, health concerns or consults.

Medication refill: $25.00

• This is for any prescription refill that has previously been prescribed by a Live Well Center Physician.  If you were not seen previously for the prescription then an appointment will be required.  

Laboratory Blood Draw: $35.00

• Includes blood draw and packaging.  Blood sample processing will be billed separately through Labcorp, Vibrant America, or Access Medical.  Processing will be a separate charge and will be the responsibility of the patient, not The Live Well Center.   

10 Minute Phone Consultation During Business Hours: $50.00

10 Minute Phone Consultation During Non - Business Hours: $100.00

• In person appointment may be required depending on medical requirement.

Platelet-Rich Plasma (PRP) Injections: $600.00 to $800.00 depending on treatment

• After creating platelet-rich plasma from a patient’s blood sample, that solution is injected into the target area, such as an injured knee or a tendon.  Studies show that the increased concentration of growth factors in platelet-rich plasma may stimulate or speed up the healing process, shortening healing time for injuries, decreasing pain and even encouraging hair growth.
• PRP treatment can help support wound healing in trauma and joint injury. The technique can address male pattern baldness, stimulate the growth of hair transplants and enhance other cosmetic procedures.
• The results of PRP treatment are most noticeable after several weeks for joint injections and six months for scalp injections, and are not permanent; patients may require additional injections at the direction of their doctor.

Wellness Coaching: $50.00/session, $600.00 Total

• A 12 week approach with a focus on the wellness of your body, mind and soul to build lasting and meaningful lifestyle change and personal transformation towards well-being.  One session per week for twelve weeks.

Home Sick Visit: $250

• 30 minute appointment.  Availability may vary.  The Live Well Center reserves the right to decline this service for any reason.
